Try:
Settings > General > Siri > On
Settings > Touch ID & Passcode > On
Settings > Touch ID & Passcode > Allow access when locked > Siri > Off

If this happens with headphones then try without headphones and then with another pair of headphones. It couod be that the headphones connectin is turning on Siri
II've turned off Siri and then the voice control chimes in and does the same thing.
I don't recall this occurring with my headphones. i am using Dre beats solo when I'm at the gym. If it's the headphones how do I know it's defective? They are only 3 months old.
The suggestion is to turn Siri on, so that it overrides voice control, but then disable Siri from the lock screen so it cannot activate when the device is locked. You'll probably want to activate auto lock if you don't already use it. If Siri self-activation only occurs when using headphones then Illaass may well have identified the issue.

How do you have Siri unable to activate when auto lock is on?
